Home BP Monitoring

The practice runs a Home Blood Pressure Monitoring Service for those patients whose blood pressure needs regular or short term monitoring.  Patients referred to this service by their doctor will receive a letter inviting them to arrange to borrow one of the surgery BP machines.  This is collected from reception on a pre-arranged date together with a form to use for recording the readings.The completed form is returned to the surgery with the BP monitor and the doctor reviews the results and decides if any further action and / or medication changes are necessary.   Patients are also required to provide a stamped self-addressed envelope so that the doctors report and instructions may be posted to them.

We recognise that some patients may prefer to purchase and use their own personal BP monitor machine*.  However, we still require the readings to recorded and submitted in the correct format and returned to us with an stamped self-addressed envelope.  Therefore a copy of the Home Blood Pressure Monitoring Record Form is available to download and print off below, together with the GPPAQ (physical activity questionnaire
